Alpha-GPC är en kemikalie som frigörs när en fettsyra som finns i soja och andra växter bryts ner. Den används som läkemedel.

I Europa är alfa-GPC ett receptbelagt läkemedel för behandling av Alzheimers sjukdom. Det finns i två former; den ena tas genom munnen och den andra ges som en spruta. I USA finns alfa-GPC endast som kosttillskott, främst i produkter som marknadsförs för att förbättra minnet.

Andra användningsområden för alfa-GPC är behandling av olika typer av demens, stroke och ”mini-stroke” (transitorisk ischemisk attack, TIA). Alpha-GPC används också för att förbättra minne, tankeförmåga och inlärning.

Hur fungerar det?

Alpha-GPC verkar öka en kemikalie i hjärnan som kallas acetylkolin. Denna kemikalie i hjärnan är viktig för minnes- och inlärningsfunktioner.

Alpha-glycerophosphocholin (alfa-GPC eller α-GPC) är en kolinerg förening som används för kognitiv förbättring och för att öka effekten hos idrottare. Det verkar också stödja cellmembranen och kan bidra till att förebygga kognitiv försämring.

Alpha-GPC (alfa-glycerophosphocholine eller choline alphoscerate) är en kolinhaltig fosfolipid. Vid intag metaboliseras alfa-GPC till kolin och glycerol-1-fosfat. Kolin är en föregångare till acetylkolin, en neurotransmittor som är involverad i minne, uppmärksamhet och sammandragning av skelettmuskler. Glycerol-1-fosfat används för att stödja cellmembranen[1].

Alpha-GPC verkar lätt passera blod-hjärnbarriären och absorberas snabbt. Det är för närvarande det bästa kolinergikumet för att öka kolinnivåerna i plasma och hjärna[2].

Vilka är alfa-GPC:s främsta fördelar?

Oralt tillskott av alfa-GPC är främst av intresse för nootropa eller kognitionsförbättrande ändamål. Det finns ett antal studier på gnagare som stöder denna effekt, men den har ännu inte visats hos i övrigt friska människor. Hos äldre vuxna med mild till måttlig demens – som innebär störd kolinerga neurotransmission – förbättrar alfa-GPC kognitiva symtom (t.ex. minnes- och uppmärksamhetsstörningar). 3. Alfa-GPC kan också förbättra effekten av acetylkolinesterashämmare (dvs. läkemedel som ökar tillgängligheten av acetylkolin genom att bromsa nedbrytningen av acetylkolin), som används för behandling av Alzheimers sjukdom. 3.

Idrottare är en annan population som kan ha nytta av alfa-GPC-tillskott. Preliminära bevis tyder på att alfa-GPC ökar kraften i vertikalhopp. 4[4][5] Dessutom rapporterades i en pilotstudie att alfa-GPC ökade toppkraften i bänkpress, men inte toppkraften eller kraftutvecklingshastigheten. 6] Huruvida alfa-GPC ökar den isometriska styrkan är för närvarande oklart. 5[5][7]

Vilka är alfa-GPC:s främsta nackdelar?

Alpha-GPC tolereras i allmänhet väl. Allvarliga biverkningar har inte rapporterats i studier på människor vid en dosering på 1 200 mg per dag i sex månader.[8] Nivån för inga observerade biverkningar är 150 mg per kg kroppsvikt per dag.

På senare tid har det uttryckts oro över alfa-GPC:s potential att öka risken för kardiovaskulära sjukdomar (CVD) eftersom det fungerar som substrat för syntesen av trimetylamin-N-oxid (TMAO) i tarmen, och TMAO är förknippat med negativa kardiovaskulära resultat hos personer med CVD och i mekanistiska studier[9].

I en kohortstudie från 2021 med mer än 12 miljoner deltagare (minst 50 år gamla), inklusive 108 877 alfa-GPC-användare, rapporterades att alfa-GPC-användning i minst 12 månader var förknippad med en ökad risk för stroke under 10 år[10]. 2021 fann man dessutom i en musstudie att alfa-GPC-tillskott främjade ateroskleros[11].

De nu tillgängliga bevisen är dock av preliminär karaktär, så det behövs randomiserade kontrollerade studier och stora kohortstudier för att bekräfta dessa resultat.

Hur fungerar alfa-GPC?

Mekanistiska bevis tyder på att alfa-GPC utövar sina effekter genom att öka syntesen och frisättningen av acetylkolin i hjärnan, där det är involverat i minne, motivation, upphetsning och uppmärksamhet[12][13][14][15].

Acetylkolin är också ansvarig för den aktionspotential som stimulerar musklerna att dra ihop sig. Därför är teorin att ökade acetylkolinnivåer leder till en starkare signal för muskelkontraktion och därmed ökad kraftproduktion.

För att mildra symtom på kognitiv försämring användes i nästan alla studier en dosering på 1 200 mg per dag, uppdelat på tre doser om 400 mg.

För att öka effektuttaget har studierna använt en dosering på 300-600 mg, kompletterat 30-60 minuter före träning.
